Intent and scope: The work under this contract consists of installation of paved driveway aprons and reworking shoulders in specified locations along State Road 44 in Lake County. Bidders must complete and submit the Small Business Affidavit (Form No. 275-000-01) for approval prior to receipt of bid documents.
